Who can Provide Psychotherapy in Ontario?

When people begin searching for mental health support in Canada, they often encounter many different professional titles. Psychiatrists, psychologists, psychotherapists, social workers, doctors, and nurses may all be involved in providing mental health care. While these professionals can overlap in some areas, their training, scope of practice, and roles within the healthcare system are different.

Understanding these distinctions can make it easier to navigate the mental health system and choose the type of support that best fits your needs. A central concept that connects many of these professions in Ontario is the controlled act of psychotherapy.

What Is the Controlled Act of Psychotherapy?

In Ontario, psychotherapy is not simply defined as supportive conversation about stress or life challenges. It is legally defined as a controlled act, which means only certain regulated health professionals are permitted to perform it when they have appropriate training.

The controlled act of psychotherapy refers to:
Treating a serious disorder of thought, cognition, mood, emotional regulation, perception, or memory through a therapeutic relationship.

Because this form of treatment can significantly affect a person's psychological functioning, provincial legislation restricts who is authorized to provide it.

In Ontario, members of six regulated health professions may perform the controlled act of psychotherapy when they have the necessary training:
College of Registered Psychotherapists of Ontario
• College of Psychologists and Behaviour Analysts of Ontario
• Ontario College of Social Workers and Social Service Workers
• College of Nurses of Ontario
• College of Occupational Therapists of Ontario
• College of Physicians and Surgeons of Ontario

This means psychotherapists, psychologists, psychiatrists, physicians, nurses, occupational therapists, and social workers can all legally provide psychotherapy when they are trained and practicing within their scope. The differences between these professionals are primarily related to their education, training focus, and the types of services they provide.

Psychiatrists in Ontario

Training: Psychiatrists are physicians who specialize in mental health.

Their training pathway typically includes:
1 Undergraduate education
2 Medical school
3 A five year medical residency in psychiatry

Psychiatrists are regulated by the College of Physicians and Surgeons of Ontario.
Because they are medical doctors, their training emphasizes biological and neurological aspects of mental illness, as well as medical treatment.

What Psychiatrists Can Do In Ontario

Psychiatrists have the broadest medical authority among mental health professionals. They can:

• diagnose psychiatric disorders


• prescribe psychiatric medications


• order laboratory or medical tests


• admit patients to hospitals


• manage complex psychiatric conditions


• provide psychotherapy

Psychiatrists often work with conditions that require medical management, such as bipolar disorder, psychotic disorders, severe depression, or treatment resistant conditions. While they are trained in psychotherapy, many psychiatrists focus primarily on assessment and medication management, especially in hospital or specialized clinical settings.

Psychologists in Ontario

Training: Psychologists are specialists in psychological science and human behaviour.

In Ontario, psychologists typically complete:
1 An undergraduate degree
2 A doctoral degree in psychology such as a PhD or PsyD
3 extensive supervised clinical training
4 professional licensing examinations

Psychologists are regulated by the College of Psychologists and Behaviour Analysts of Ontario.
Their education emphasizes psychological theory, behavioural science, and research based mental health treatment.

What Psychologists Can Do In Ontario

Psychologists are able to:

• diagnose mental health disorders


• conduct psychological and cognitive assessments


• administer intelligence and personality testing


• provide psychotherapy

Unlike psychiatrists and other physicians, psychologists cannot prescribe medication in Ontario. However, psychologists often specialize in complex psychological assessments such as ADHD evaluations, learning disability assessments, and neuropsychological testing.

Registered Psychotherapists in Ontario

Training: Registered Psychotherapists specialize specifically in psychotherapy and therapeutic treatment. In Ontario they are regulated by the College of Registered Psychotherapists of Ontario.

Their training typically includes:
• a graduate degree in counselling or psychotherapy


• formal education in therapeutic models


• supervised direct client work


• professional registration examinations

Psychotherapy training focuses heavily on therapeutic techniques, emotional processing, trauma treatment, and relational dynamics.

What Psychotherapists Can Do In Ontario

Registered Psychotherapists provide mental health treatment through structured therapeutic conversations. Their work often focuses on:
• emotional regulation


• trauma recovery


• relationship patterns


• behavioural change


• identity development and life transitions

Psychotherapists cannot prescribe medication and generally do not perform formal psychological testing. Their work often involves evidence based therapeutic approaches such as Cognitive Behavioural Therapy, Dialectical Behaviour Therapy, Emotion Focused Therapy, Acceptance and Commitment Therapy, and trauma informed approaches.

*Registered Psychotherapist vs Registered Psychotherapist (Qualifying)*

When searching for therapy in Ontario, people may notice the titles Registered Psychotherapist (RP) and Registered Psychotherapist Qualifying (RP Qualifying). These titles represent different stages of professional registration.

Registered Psychotherapist (RP): A Registered Psychotherapist has completed all requirements necessary to practice independently. This includes:

• completing graduate level education in psychotherapy


• completing hundreds of supervised clinical hours with clients


• meeting all competency requirements for the profession


• passing the professional registration examination

Once these requirements are met, the therapist can practice independently without mandatory ongoing supervision.

Registered Psychotherapist (Qualifying): A Registered Psychotherapist Qualifying has completed the academic education required for the profession but has not yet completed all requirements for full registration. This means they:

• have not yet accumulated all required clinical practice hours


• have not yet passed the professional registration examination


• must practice under regular clinical supervision from an experienced practitioner

This designation represents a transitional stage early in a therapist’s professional development. Both RPs and RP Qualifying therapists are regulated by the College of Registered Psychotherapists of Ontario and must follow the same professional standards and ethical guidelines.

What Registered Social Workers in Ontario Can Do

Another group of professionals who frequently provide psychotherapy are Registered Social Workers (RSW). Social workers are regulated by the Ontario College of Social Workers and Social Service Workers. When social workers receive clinical training in psychotherapy, they are also authorized to perform the controlled act of psychotherapy.

Social Work Training and the Person in Environment Perspective

Social work education is grounded in the person in environment perspective, which emphasizes that mental health cannot be separated from a person’s life context. Social workers are trained to understand how psychological well being is shaped by factors such as:

• family relationships


• trauma history


• housing and financial stability


• culture and identity


• access to healthcare


• systemic barriers


• community resources

Because of this training, social workers often approach therapy holistically, integrating psychological treatment with an understanding of social and environmental conditions that affect a person’s life.

Master’s level social workers with psychotherapy training can:

• provide psychotherapy


• support trauma recovery and emotional regulation


• address relationship and family dynamics

• help individuals navigate healthcare and social systems


• connect clients to community resources


• advocate for supports that affect well being

This combination of psychological and systemic training allows social workers to support both internal emotional experiences and external life circumstances.

Physicians and Nurses Providing Psychotherapy in Ontario

In addition to psychiatrists, other physicians and nurses may also provide psychotherapy when they have appropriate training.

Physicians: Family doctors and other physicians are regulated by the College of Physicians and Surgeons of Ontario.
Some physicians receive training in therapeutic approaches and incorporate psychotherapy into their medical practice. They may also provide mental health assessments, prescribe medications, and coordinate care within the healthcare system.

Nurses: Certain nurses, particularly those working in mental health settings, may also perform the controlled act of psychotherapy.
They are regulated by the College of Nurses of Ontario. Mental health nurses may provide therapeutic support, counselling, and structured psychotherapy interventions depending on their training and role within healthcare settings.

Choosing the Right Mental Health Professional in Ontario

Each profession contributes different expertise to mental health care.
- Psychiatrists and physicians often focus on medical assessment and medication management.
- Psychologists frequently specialize in psychological testing and evidence based therapy.
- Psychotherapists focus specifically on structured psychological treatment through therapeutic relationships.
- Registered Social Workers combine psychotherapy with a broader understanding of life circumstances and systemic influences.
